Mature hands are hands that appear older generally do so because of consistent unpredictable changes have occurred. The constellation of mature hands consist of loss of fat on the back of the hands making the hands appear somewhat veiny or very visible bones and tendons. In addition to loss of volume is the thinning of the skin and a crepy wrinkled appearance combined with brown spots or sunspots and photo-degeneration.
Much like facial rejuvenation, hand rejuvenation is usually involving a multimodal treatment. The restoration of hand volume with the use of injectable soft tissue fillers such as Voluma, Radiesse, or Sculptra can restore volume to the back of the hand. The use of patient’s own fat can also be injected in the back of the hand to great success. With the restoration of volume one can also then treat the crepy skin with fractional laser resurfacing using fractional carbon dioxide or even fractional radio frequency energy. And, finally, the problem of sun damage and
sunspots can be treated with intense pulsed light and/or pigment lasers.
With this multimodal therapy generally one can restore a much more youthful appearance to the hand much more matching the appearance of the face and neck.

Loss of volume with age causes the veins and tendons to become more prominent. Skin changes are also a factor with sun damage and thinning accentuating the aging look. Restoring volume with fat injection is a permanent way to hide tendons and veins without interfering with their function. Stem cells which are contained in the injected fat will migrate into the deeper layers of the skin and improve skin quality and texture. Any remaining sun damage can be treated by a variety of options including laser resurfacing.
We often use fillers in the hands to reduce the look of aging as they will look plumper. Also the use of lasers to reduce the brown spots and sun damage can be very helpful.
